Liability Release Form

Each client is asked to sign a liability release. Here is the release for you to review:

*New clients will get an email to fill out Medical History/Liability Release. This paperwork can also be completed at the first visit. If filling out paperwork at our location please plan to arrive 15 minutes early for your appointment.*

CANCELLATION/NO SHOW POLICY - We understand that life may occasionally get in the way of your appointment. If you do not show up for an appointment and did not call 24 hours in advance to reschedule or cancel, dermani MEDSPA® reserves the right to charge $50 to your credit card for this inconvenience.
